Stutter/lag every 2 seconds, but only at sea

It's not really a bug and there's no tech support forum, so hopefully this is cool here.
I've been having the weirdest issue since I upgraded my video card (1080ti) (and of course it was the same day a big windows update a couple days ago so I can't rule either out) where there's a consistent stutter every 2 seconds exactly, but only at sea. It's just a few frames dropped but it's oddly annoying. It doesn't really feel like a hardware issue since no other game does it at all and on land it's perfectly smooth. It's just at sea, and only in this game. I've been trying for days to figure it out and haven't found or fixed anything. Anyone else had something similar happen? Here's what I've tried so far:
Alternate Vsync/Gsync/off, both in game and in nvidia control panel.
Tried all those variations alongside frame limiting at 60/120/144 fps.
Tried multiple variations while setting my monitor's refresh rate between 60/120/144.
Turned off HPET in bios
Turned on CPU overclocking in bios
Turned on ram overclocking in bios (can only have one on at a time, ram or cpu)
Tried various resolutions, full screen and windowed.
Turned graphics all the way down, and all the way up, and multiple variations of graphics settings.
Tried each setting in the config startup menu (Back buffer count, device reset at scene, 3d background control, 3d background control draw level)
Tried running single display (sometimes multi-display causes frame issues)
Used MSI Afterburner to turn power limits up.
Used MSI Afterburner to ensure card wouldn't idle and verified full clock speed
Turned nvidia power settings to Maximum performance and prefer single display
Turned windows setting power profile to maximum
Tried various hardware audio configurations
turned off win 10 xbox recording
Uninstalled nvidia experience
Updated to newest drivers
Tried a few older drivers
Ran DDU for a clean video driver install
Turned off as many background processes as possible
Turned off windows defender
Changed windows tick timing
...I really can't think of anything else to try. It's so weird too that no other games are having a stutter issue. It's only UWO, and only at sea. Anyone else have something similar happen and know an arcane ritual to fix it, or is my computer just unique in it's ability to have trouble running a 10 year old game? My gpu's only at like, 8% utilization, it's not like it's exactly straining here.

Still been occurring just the same since I made the OP. Thanks for the ideas. I'll try making a support ticket, maybe there's some totally weird-ass compatibility issue like with my specific gpu/mobo/ram/video card combo and something in the game they'll notice or something. That is a thing with PC games and wouldn't surprise me at all actually.
